Level 152 is the 153rd positive level of the Backrooms.
Description
Level 152 is an extensive, labyrinthine network of hallways and rooms, all sharing a highly sterile and artificial aesthetic. The environment has often been described as "a fusion between a hospital and a lab" due to its pristine, sanitized nature. The walls are composed of smooth, white composite panels, while the ceiling is lined with fluorescent lights. The floor is a seamless, glossy green surface that reflects everything with unnatural clarity. The excessive cleanliness of the level has been attributed to an unknown self-purifying mechanism embedded within the structure, continuously eliminating dust, debris, and biological matter at a molecular level.
The atmospheric composition of Level 152 is as follows: 78.1% nitrogen, 20.2% oxygen, 0.9% argon, 0.3% carbon dioxide, 0.5% traces of unknown chemical agents. The temperature within Level 152 averages around 23.4°C (74.1°F) but can fluctuate between extremes of 18.2°C (64.8°F) and 27.9°C (82.2°F) depending on unexplained environmental shifts, likely influenced by the automated climate control system embedded within the walls.
The corridors are mostly empty, though occasional remnants of past wanderers, such as torn garments, abandoned notes, and fragmented medical equipment, can be found. Interspersed along the hallways are rooms containing a variety of unnerving elements. Some rooms resemble examination chambers, with biohazard containment units, flickering computer terminals displaying unreadable data, and steel tables stained with unknown substances. Others contain seemingly ordinary hospital beds, though prolonged exposure to these areas often results in auditory hallucinations, primarily consisting of whispered conversations and the beeping of non-existent monitoring devices.
Windows are present throughout the level, but they do not display any recognizable external environment. Instead, they reveal a void of shifting organic patterns resembling pulsating tissue. Some have reported that the patterns occasionally take on humanoid shapes, pressing against the glass from the other side. Attempting to break or open these windows results in the wanderer immediately vanishing without a trace, their fate ultimately unknown.

One of the most disturbing locations within the level is the operating room. These rooms contain a single metallic bed surrounded by motionless surgical arms mounted to the ceiling. The walls and floor are also colored differently, being white and gray respectively. If one lingers for too long, the arms will activate and attempt to perform "reconstructive procedures" on the subject, often resulting in fatal alterations. Those who survive report extensive scarring, unnatural joint realignments, and the insertion of foreign metallic components within their bodies.
Entities
Level 152 hosts a variety of entities, including Facelings, which appear as scientists or doctors in white lab coats. While some Facelings perform repetitive, meaningless tasks—such as writing gibberish notes or adjusting blank computer monitors—others will attack on sight, using scalpels or syringes filled with unknown substances to incapacitate victims. Other entities present include Hounds, Dullers, Clumps, Bursters, Skin-Stealers, Crawlers, Insanities, and Camo Crawlers.
There is also a level-exclusive entity known as The Recreation. The Recreation is a grotesque, flesh-covered mass that reproduces asexually, generating multiple smaller versions of itself through a budding process. Each instance of The Recreation appears as a malformed humanoid encased in a layer of translucent skin, within which several pulsating organs are visible. Autopsies of deceased specimens have revealed extra lungs, redundant nervous system clusters, and misplaced muscular structures. These entities are highly aggressive, lashing out with elongated, bladed fingers that secrete a corrosive fluid capable of dissolving human tissue upon contact.
Colonies and Outposts
No known colonies or outposts exist within Level 152. Due to its inherently hazardous environment and the unpredictability of entity behavior, establishing a permanent settlement is considered impossible.
Entrances and Exits
Entrances
Level 152 can be accessed from Level 117 or Level -28 by locating and passing through a green science lab door that occasionally appears in those levels. Another, significantly rarer entrance exists within Level 704, requiring one to locate a slimy, pulsating door. This entrance is considered highly unstable and may not always lead to Level 152.
Exits
Exiting Level 152 is possible but highly challenging. One method involves locating a deteriorated office room that contains an array of outdated computers and medical files. If a wanderer accesses the correct computer terminal and navigates through its cryptic system, they may find themselves transported to Level 0, Level 1, Level 4, Level 39, or Level 153.
Another means of escape involves discovering a hidden service elevator, which, if functional, can transport wanderers directly outside a random building in Level 11. There is also a reinforced door marked with "Botanical Research Facility", which can lead to Level 23 or Level 115. However, this door has been known to disappear or change locations without warning.
Lastly, a particularly ominous exit exists in the form of an unmarked morgue door. Entering this doorway will result in immediate transportation to Level 666, a destination best avoided.
